Our Mission
Journey Care Human Services was founded by Yvonne Camacho Carrasquillo, whose personal journey navigating healthcare systems, public benefits, and support services for her own family inspired the creation of the organization. Through these lived experiences, she recognized how difficult it can be for families to access essential resources. Journey Care was created to guide others through those challenges with compassion, advocacy, and real-life understanding.

Our mission is to provide compassionate, community-based care coordination, benefits advocacy, and health navigation services that empower individuals and families to access essential resources, overcome barriers, and improve their overall well-being.
