The Journey Behind Journey Care

 Journey Care Human Services was founded by Yvonne Camacho Carrasquillo, whose personal experiences navigating healthcare systems, public benefits, and social services inspired the creation of the organization. 

 As a mother and caregiver, Yvonne spent years advocating for her sons and family members while navigating complex systems that were often difficult to understand and access. Through these experiences, she encountered many of the same challenges that thousands of families face every day—confusing processes, limited guidance, and barriers to essential services.

Rather than allowing those challenges to define her family’s journey, Yvonne turned them into a mission to help others.

 Journey Care Human Services was built from those lived experiences. Every service offered by the organization—care coordination, benefits advocacy, health navigation, behavioral health support coordination, and community referrals—reflects real situations Yvonne personally faced while helping her own family access the resources they needed. 

Because of this firsthand understanding, Journey Care approaches every client with empathy, patience, and genuine care. The organization was created to ensure that individuals and families do not have to navigate these systems alone.
